HOW TO APPLY

Applicants who wish to attend LISA High Academy for the 2012-2013 schools year must submit a completed application to the admissions office preceding the start of the school year. The student enrollment calendar for each school year will be announced on the LISA High School web site. Online applications are also available during the enrollment period.

Parents/guardians wishing to enroll their child in LISA High Academy should provide the following:

• Copy of birth certificate or passport
• Proof of address (lease agreement, current utility bill or personal property tax bill)
• The child’s immunization record
• Social Security #
• Previous school transcripts

High School students will be required to meet with the guidance counselor prior to acceptance regarding their course selection.
